I've read where a true attack accompanies new lesions, but MRIs do not always eveal 'active' lesions.
During my major attack, nothing lit up on my MRIs, but I was surely a doosey. It's not as if I wasn't MRI'd in good timing to catch the buggers. But, nothing lit up.
You raise a great question because when you read the literature it points to active demylenation and attacks though for many this does not hold true.

I am wondering what the definition of an acute relapse is.......and how long they last for everyone as I am sure this is completely diferent for us all. I am in my fifth week of what my neuro said was a severe relapse..and had two new active lesions I think showing on MRI. Not sure that really answers your question..but it is my experience...and I am not in a wheelchair. Just VERY EXHAUSTED!
