MRIs are much better than LPs for assessing MS/no MS. Contrast is definitely a good idea, particularly if you are in a flare now. New activity will show more clearly. However, comparison MRIs, whether or not contrast is used, are something you should ask for.
If this neuro stays in wait-and-see mode after this next assessment, find a new and better doctor. I can't imagine what this one is waiting for.

Yes, I would agree a second opinion is in order, preferably from a reputable MS specialist if you can access one.
I agree with ess and db about an MRI with and without contrast. I wouold seek out another neuro for a 2nd opinion as well , it would seem 20 lesions would point toward a dx other than migraines.
Have all the mimics been ruled out? Lyme (Western Blot), Sjogrens, ANA and other rheumutological tests for lupus, etc?
LP's do not make or break an MS dx. Did you see Quix's post "Lies My Neurologist Told Me?" it has loads of information as doe the Health Pages located at the top of this page next to the yellow icon.

I have only had Lyme ruled out and an EEG for Epilepsy..He did not tlk about other tests other than LP the last time...(that's why I got so darn confused)
No blood work taken..just MRI, EEG...
