Without the ability to examine and obtain a history, I can not tell you what the exact cause of the symptoms is. However I will try to provide you with some useful information.
An EMG can differentiate ALS from other neuromuscular disorders. If the test was limited to test for carpal tunnel, ALS may be missed. Carpal tunnel typically affects the fingers/hands. Pain on the outside of the arm (upper or lower arm?) is a separate nerve but needs to differentiated from the anterior interosseus nerve branch of the median nerve.
Did the EMG suggest carpal tunnel at the wrist?
I would advise you to discuss your symptoms with your primary physician. You may need a referral to a neurologist to fully evaluate.
Thank you for this opportunity to answer your questions, I hope you find the information I have provided useful, good luck.
