Yes they can mistake it for a miscarriage. It happened to me at 6 weeks. I was told by an ER doctor AND my obgyn that I had a miscarriage. They couldn't see my baby on the ultrasound because there was such a big blood clot in my uterus. I had a lot of bleeding and cramps for one evening, then spotting after. I even discussed scheduling after D&C with my doctor. After my blood hcg levels came back they realized my levels increased. My dr suspected I might still be pregnant. He sent me to a specialist who did a detailed ultrasound and sure enough her little heart was beating strong. My bleeding was caused by a hemorrhage or hemotoma under the placenta. I was on restrictions for several weeks until the blood clot desolved. I'm now 26 weeks with a healthy pregnancy :-)
It's possible but they look for other signs as well like low or decreasing hcg level severe cramps clots in the blood there are a lot of symptoms they look for b4 saying miscarriage

It is a lot of bleeding. They just call it a hemorrhage because it is a lot of bleeding that doesn't stop immediately.
