I have had a tear in my esophogus. It was from vomiting then the stomach acid wearing on my esophogus. I went to urgent care because it worried me. I ended up on an h2 blocker to prevent it from happening again. If your Dr has an on call number, call. Get a consultation on what the Dr wants you to do if you can. Its very hard to say what it could be.

I would get really bad nose bleeds and my sinuses would get really dry. So when I threw up it would make it bloody. But I knew what it was. If you are having this go to er or walk in clinic

How much blood?  If you are vomiting red blood you could have a bleeding ulcer and that is an emergency.   I have had a tiny amount before and it was due to irritation in my throat.  You should at least make an appointment with your physician.   If you vomit red blood, become dizzy or faint or have black sticky stools you should go to the emergency room.
