Health insurance
So i got a form today from the insurance i applied for. it came with a MC 210 state of facts sheet but i don't understand how to fill it out. does anyone know how to? it would mean so much if someone can please help me on this. my insurance well temporary insurance expires this month so i need to send this in asap so i can get it asap.
i know. i got everything else done just that stupid paper. lol. i'll go into the office and have them help me there since they won't answer my calls lol! thank you very much! :)

girl i have called that number from tuesday-friday at least 5 times a day. they had me on hold for 30 minute and then told me to call back tomorrow or when a worker is available! I'm like ughhh seriously. so i have been asking anyone lol. I'm so desperate i have googled and nothing. but i'll call monday and keep trying! thank you :)
