I've had it both ways. My first i spotted a little bit and then it got really bad and i had huge clots, that was when we got to the hospital. Then the cramps got really bad and we had an emergency d and c that day.  My second I spotted for a few days and then I knew to get to the ER so we went and found out through an ultrasound that time.
Generally most women look for spotting with cramping before calling their doctors or going to the ER. For myself i kept the general rule that if i spotted for a week straight, very light spotting, it was cause for concern. It all depends on your body though, everyone is different. but cramping and bleeding are usually the first signs. Then the doctor will ask how your symptoms are and if you had any ever and if they have gone. Then they will usually try to hear a heartbeat and if nothing go for an ultrasound.
I hope you aren't having to go through this and are just curious.
