PSA levels

a psa level of over 300 in a 45 y/o male, is that always indicative of prostatic ca
what other problems might cause a level this high

My father in law was diagnosed with extensive metastasis prostate cancer..He had his testicles removed and put on hormone therapy.  This past month his PSA results showed a spike from 5.0 to 260 and in less than a week a retest showed even higher numbers, it spiked to 300 in less than ten days...

What is happening here? why this sharp abrupt spike?

Thats the most obvious.     Even enlarged prostates or infection are not that hight.     At that level,   a biopsy should be done as soon as possible.
