Gonorrhea or NGU? Unsure

Straight white male 32 years old. I have occasionally had unprotected oral sex with other males, and had contracted gonorrhea in the past. I was treated for it, and it went away. Irresponsibly and stupidly, I did this twice. You'd think I'd have learned a lesson but of course I did not. However, I was much more carefully selective of partners.

After receiving unprotected oral sex from a friend, quite quickly I noticed familiar symptoms emerging, but this time around seems different:

Lower abdominal pain just below waistline, Frequent urge to urinate, sporadic "electric shocks" or dull stabbing pains along penile shaft, my penis itself feels stiff and pulled in/smaller (when flaccid) which could be due to anxiety. I do not notice any burning sensations before, during or after urinating. I did not have any discharge until 5 days after noticing first symptoms, and it is VERY minimal and clear. I expected much more discharge to appear in the AM, but none appeared. Only a very small amount after "milking" it out. Occasionally I will get the feeling that something is "passing" along my urethra or as if my penis is release a small amount of urine. However, upon checking, never is. There is the occasional tingle here and there. No alarming redness, swelling or bumps. Testicles feel fine although may sometimes feel an ache from the abdominal pain. Overall, the symptoms are mild, but present enough to be noticeable and concerning. I can just tell there is something off going on down there.

The prior time I was treated for gonorrhea, I was prescribed cefixime and zythromax. I bought a 50ml bottle of the cefixime so I had plenty of it left over. After careful reading and studying during this current symptom flare, I decided to take a 200mg dose of the cefixime. That was on day 4 of symptoms. Now, two days later, the symptoms are only slightly less. But, progress is progress. I am currently in monitor mode to see if these symptoms go away and am abstaining from any sexual contact. (And, have 100% sworn to never again do the behavior that caused this.)

After reading through many of these threads, I'm curious about NGU. While I do not have the burning sensations before, during or after urination, I feel like I may be exhibiting signs of this issue. I also thought it could be adenovirus, as a week prior to symptoms starting, I did have a very minor cold in which my throat was sore and I had very bad congesting. However, that went away rapidly and I'm not sure the two issues are related. I have had zero throat or eye problems during these symptoms, so I feel it's safe to rule out adeno.

I'm uncertain as to what the cause of my lower abdominal pains are. They feel like low dull aches or like muscles tensing, in my pubic area below my waist line. I am aware of genitally focused anxiety, but there are definite physical things happening. I understand this is a very long winded description and I appreciate anyone's patience for reading through it.

I'm considering taking another dosage of cefixime but I'm unsure if this is wise or not as I've read the CDC has advised against even using it for gonorrhea anymore, but my doc did prescribe it. I have been drinking plenty of fluids and taking a good amount of vitamin C. I guess I'm just unsure as to whether or not this is gonoccal or ngu, but as I said for now I am just monitoring to see if symptoms lessen over time. Any thoughts, advice or suggestions would be greatly appreciated.

Hey man. Just updating.... finally got to the clinic and provided urine and blood for std tests. The doc said everything looked great and he didn’t feel anything abnormal during the exam. The results will be back in about five days he said. He said that chlamydia can be a tricky bacteria but I’m hopeful that it’s not chlamydia causing my symptoms.

Since we last wrote, my symptoms have pretty much dissipated except the occasional ache/burning sensation about one inch up and to the left of my penis. I’m hoping it’s a mental thing or just a muscle that is getting overworked somehow.

How have your symptoms been?  Did the negative test results let your mind relax a little and let the symptoms you felt go away?

Yes it did and my symptoms have gone away. I'm confident that during the time I submitted a sample for testing I either had some kind of infection or gono/chlamydia, too. Since everything was negative, I'm not sure what was going on.

My situation being exposure from occasional mutual oral left me to believe that either I've developed some kind of allergy or body reaction to foreign saliva, anxiety driven symptoms, ngu or just a UTI.

The only symptom that lingers now (about four days after getting test results) is a strong urine smell occasionally. But, that too could just be me being hypersensitive. Everything else is fine, including my penis not feeling tensed up and extra small (back to my normal flaccid size thank f'n god lol).

If/When you get  your results, and they're normal, grill the doc about ngu/ureaplasma/mycoplasma etc.

Update 10 days after first symptoms appeared:
Spoke with the doctor and my results were negative for gonorrhea and chlamydia. He was not worried about syphilis or herpes either, and was sort of at a loss as to what the problem could be. He thinks it could be NGU and suggested I see a urologist.

I will have to set up insurance to do this. The symptoms are very weak now but still there just enough to have me worried. I feel like I have more questions than answers at this point.

Wondering about NGU, Mycoplasma, Ureaplasma, etc.. the stuff that a regular clinic doc doesn't know too much about.

Next day: Pelvic pain still persistent. Still no discharge or burning when urinating. Sporadic stabbing penile pains.

UTI? Hope someone can shed some light.

Read my other posts. Your second go around sounds similar to mine. Since the end of March, I have been dealing with mine and have been unable to test due to geographic location. I have had an std before and got it cured and I think my anxiety is getting the best of me trying to convince myself that I have something. I have noticed that even a few beers helps. But it won’t cure me. I need to get tested to really get piece of mind.  Thoughts?
I just read your other post and it sounds as though we have the exact same thing going on. It is extremely frustrating.  I think I'm just going to go to the doctors today to see what the hells going on, because I'm tired of these little symptoms coming back, even though it was my fault in the first place that they are there.

I've read so many different threads on here and at this point it seems like I could have any number of issues going on but it's impossible to put a finger on it until I see a doctor. It's hard for me to see a doc as well due to work and money but I'm gona have to suck it up and go.

I honestly don't think it's anxiety.
Spoke with doctor, said my results for gonorrhea/chlamydia were negative. He suggested I visit a urologist and that it may be NGU.

So now there's that I have to deal with.
Interesting. Did he run any other test or just those two? And the reason I feel like mine is anxiety is because I know what it felt like before and when I convince myself that I don’t feel bad, I feel them within hours or the next day. They will come and go the more I search for specific symptoms. I think if I could just get to  A doctor and get negative tests, I could convince myself that these are just normal body sensations and the hypersensitivity would go away.

Did he say what the recommended treatment for NGU is?
No he didn't. I mean it could be anxiety. Doing these stupid things we do definitely takes a toll on our psyche. But you and I both know that when you feel something happen randomly without even thinking about it, it's real and there. Maybe the anxiety kicks in at that point and exacerbates the symptoms.

I wish someone with confirmed positive results for NGU would weigh in here.
Yeah man. I hear you for sure. I know that the body will send weird pains now and then. I have felt them before this encounter. I think mine in the groin region are exacerbated by anxiety because I workout quite a bit and I am taking regular muscle fatigue for something that I don’t have.

And as soon as I felt something that is normal, before, I would write it off and pay it no mind. Now, I will sit and immediately turn to this website and start trying to pinpoint the exact sensation I just had and it snowballs from there.

At the end of the day, I know that I took highly effective medicines after the encounter and the “symptoms” neither gotten worse or better. They kind of jump around I feel.
Exactly. I sit on my *** for the majority of the day at work and that definitely doesn't help. As soon as I get home, I feel much better.

I do feel a little relief after talking with the doc and him telling me it's not any of the normal stds, and not to worry about them. That at least narrows it down to a few select things it could be, which doesn't give me too much peace of mind but at least I have a better idea of what I'm dealing with.

If it isn't that other stuff (ngu, mycoplasma, ureaplasma, etc), then my assumption is that I'm somehow having allergic reactions to other peoples saliva. I've only ever done unprotected oral sex with "strangers". I always grill the people I meet and ask if they're tested, but clearly you just cant trust anyone. Which is why I'm out of that game for good now.

Today symptoms have been much better, though. Only slight dull aches in the pelvic area here and there. And I don't feel like I need to **** every three seconds. I guess if it gets any worse, I'll see a urologist.  

Still very worried about having sex with my wife, though. I don't want to transfer anything to her. But I can only hold off for so long :/
Yeah. I feel a lot better after talking to you and hearing your results are negative. When I initially took the meds and read what the professional forums were saying, I felt fine and had sex with my wife 10 days after taking the meds. And then I went away on a business trip and started googling **** again because I couldn’t shake the fact that I had done that to my wife. So then the anxiety related symptoms started coming back and made me worry all over again. She hasn’t complained of anything and his been almost 6 weeks since we initially had sex after my meds.
Since you're not able to go test or anything, i would suggest you at least try to get a lot of cranberry juice, vitamin c
And, take like a half a tablespoon shot of apple cider vinegar daily or every other day.

Update 4 days later. All symptoms essentially gone save for the occasional tingle or dull ache. Maybe this was a UTI and the cefixime cleared it up? Should I be concerned if symptoms appeared, and then went away?

Still monitoring.

Update: The morning after posting this thread.
Had a morning erecting for the first time since symptoms appeared.
No obvious white, yellow or green pus-like discharge upon manipulation when I went to the restroom.
Very slight and minimal amount of clear fluid, but nothing else.
Urinated normally, no foul smell outside of normal first morning urine.
Pain in pelvic area feels distant or non-existent, possibly my mind is searching for the feeling.

This is sort of typical for the morning, though. I feel like the symptoms come on as the day goes on.

While laying down during the night I did notice what felt like tingling or burning in my hands and feet but I think this is psychosomatic after reading some of the other threads here about similar issues.

Nothing else to report until later.
