Don't know what to do..

Well, my title states it all. I am very clueless right now. Lets start of with me. I'm a 20 years old male, living in Malaysia. I had several gay exposures, including anal and oral sex. It took me a year to realize my mistakes and now trying to get back to the life i had last time. So, I had been tested for HIV already, and it came back negative (thank God). However, now my biggest concern is to get tested for STDs. Malaysia is a country where homosexuality are banned in the laws. Anyone caught can be prosecuted. Hence, I have no idea where to go to for a full STD checkup. it is simple for STDs detected via blood test (herpes and syphillis) but other stds such as gonorrhea and chlamydia, which requires swab from the infected site.. I have no clue how to explain a swab at the rectum. I don't know how to explain to the doctor. Probably I'm posting at a wrong forum (sorry), but anyone could share some thoughts?

No, I am not sure what long term STD's in that area would due if left untreated. I can say that some STD's over time can be treated by the body. But I guess the germ thing would be the only option or some kind of lie to get the testing done.

Well, I'm sorry if I sound so stupid in this. I guess I have to search harder. :) I know STDs may cause sterility, but what if I had oral or rectal STDs? Will it still cause sterility?

I can't tell you what to say or do in this case. I don't think anyone can. Maybe tell them you are a germ-a-phobe and need these tests done.
