Can I have an std without bacteria in my urine?

For the past 6 months it is difficult to push out my stool and I have had itching around my anus. Recently I have also experienced pain in the tip of my penis. I don't have any discharge.  My doctor also tested my urine and said there was no bacteria in it...so he didnt test for any std's. I cant help thinking I might have something though. I had unprotected sex about 18 months  ago with a girl who said she is clean and gets tested every 6 months. But maybe she lied. Can anyone help? It is just an infection of different body parts that are unrelated?
