If it's not a fungal infection on the penis, what is it?

I've had this irritation in the area under my foreskin for over a year now. A fungal cream seems to keep it at bay and I've even taken Diflucan/Fluconazole (200 mg daily) for a week. They seem to help but yet it comes back. It's manageable and I can masturbate (with some temporary redness and slight burning sensation). However, if I engage in activity with someone else, it's usually not as gentle so I'm assuming I get tears and there are traces of blood and it does hurt quite a bit. But it always heals within a day.

I've been to a doctor who wanted me to test for HPV. The thing is, it's expensive and I understand there is no current HPV test recommended for men. Also there is no cure and it's supposed to clear. So I don't see the point. Also if my penis looks normal, it can't be as serious as HPV/warts could it? What else could I do please?
