Urine Test STD

Have endured irritation in urethra (tip of penis) for three weeks, with no discharge or obvious trauma.  Had difficulty urinating and irritation from friction with clothes...all day, especially when working out.  Saw my GP two weeks ago and told him about symptoms; he asked if there was any reason it could be STD...I have not had any new partners and in monogamous relationship, so I hope it's not STD.  He didn't see anything from examination and did general urine test which came back negative for Nitrates and Leuko.  Said it was probably prostitis and gave me 7 days of Cipro.  Last week I had physical, we discussed the previous test and he told me culture came back two days later with "low grade UTI".  He again took urine and report came back negative.

The irritation subsided and I felt normal enough to have regular vaginal sex with my girl (all we do, nothing anal) late last week.  Nothing crazy or hard that would cause any trauma.  The irritation came back the next day and has gotten progressively worse...so bad I cannot wear my normal jockey type underwear and have to now wear boxers.  Yesterday morning, the irritation has worsened into significant "pinching" pain in the tip of my penis.  No discharge, no burning when I urinate, but it hurts like hell.  I've started taking ibuprofen and today it's a little better but still very much there.  I have not strayed and honestly don't think my girl has but the Cipro is not having any effect.

Would standard urine test show evidence of STD?  Would a bacterial infection such as Gonorrea or Chlamydia show positive for Leuko or Nitrates?  I fully understand that identifying either pathogen requires specific test, but can standard urine test at least signal any potential bacterial infection?  Also, if my Doctor tells me I have a low grade UTI, would STD possibly fall into that category?  Would an STI that is causing me this much discomfort not show up in urine test or only be considered a low grade UTI?  I'm in long time monogamous relationship, know I have not had any other partners but certainly don't want to begin to imagine my girl has been unfaithful.  Any advice would help; I really don't want to go for STD test but confused about the situation.

Vance, can you define "low grade" UTI in men?  I've never had anything like this in my life.  Worse thing I can do, which I've done is start googling every possible thing on the net.  Seems like most everything I read with these given symptoms is STD...really haven't seen anything but STD's relating to this in men.

A standard test might show some bacteria. I don't know if it would show as a low grade UTI. Would not hurt to test for chlamydia at this point.
