Positive TP-PA from Urgent Clinic, PCP wants to retest?

Good Evening,

Long story short. Last February 2014 I received a positive RPR with a titer of 1:2 for syphilis. The PCP at Kaiser ran a TP-PA and it came back non reactive. All sexual encounters since have been condom protected. This past March, I requested HIV, HSV, and syphilis testing at an urgent clinic. The NP at this facility had treated me for BV this past February, and I was tested for common STI's  like gonorrhea, chlamydia,  and common infections like trich and yeast.

The urgent care clinic did run a Treponema pallidum AB, IF and it came back reactive for syphilis. My PCP believes it is a false positive and argues that the Treponema pallidum AB, IF is not an optimal test. Kaiser wants to use the Treponema pallidum Particle Agglutination Assay; TPPA, just to make sure.

My concern is symptoms. I do not remember any chancre sores in my vaginal region or mouth. I also have not broken out in a rash. Are symptoms noT blatantly present in women?

The NP at the urgent care clinic has no doubt that I have syphilis and it may have been overlooked in the past. However, my PCP believes that with my sexual practices and history, my odds are low for syphilis and she thinks I may have an auto-immune disorder.

This is annoying!  If anyone knows about syphilis testing, please let me know.

Hello There,

My third and final test with the TP-PA came back non-reactive. My PCP made it clear that may test positive on the RPR for the remainder of my life, however if I keep clear records, I can explain the situation. Which is good because I would like to donate blood in the future.

With that syphilis tier number I am going to go along with your PCP as this is probably a false positive.

The TP-PA is run after a positive RPR and when the TP-PA is negative it is deemed that the RPR was a false positive.

Ok, the test the urgent care clinic gave is the (FTA-ABS). Please let me know if this test does yield a high rate of false positives.

I do apologize to being anxious. I have just left an a-hole of a man I am happy all my other STI tests have come back negative.
