Yes it could be herpes. I'm not sure why the provider you saw thought syphilis instead of herpes. Would be worth asking.
you should've seen results in 48 hours after taking the medication, not it getting worse.

2) if it was syphilis.... how long is suppose to take for the ulcer to completely heal after i took the 2 grams of azithromycin?
There is a lot of resistance to azithromycin and it's no longer the preferred treatment.
I'd see a different provider if at all possible.
This is already scabbed so too late for herpes testing but you can get a herpes igg blood test now and repeat it in 3 or 4 months.
