Penis becomes erect its appears to be chapped and cracked

I have diabetes, but in control. Im taking metformin 500mg three times a day. My problem is my penis foreskin is too tight and its not stretchier now when penis becomes erect its appears to be chapped and cracked. When I peel foreskin to back its bleeds & scabs. It is a little painful and very itchy. please give me solution for this matter. If I go to the Doctor which type of specialist doctor should I choose?

Hello,
It can be due to dry skin or balanitis. Balanitis is an inflammation of the glans (the rounded head) of the penis and may be related to yeast infection or poor hygiene. What you need to do is consult a dermatologist and get a KOH examination of the skin scrapings of the penis and a biopsy of small skin sample done if the symptoms persist. KOH examination of the area will clarify whether it is balanitis due to fungal infection or not.
Also use water based lubricants during sex.
It is very difficult to precisely confirm a diagnosis without examination and investigations and the answer is based on the medical information provided. A urologist.  

Sugar control is very important, of course.  You could try Lotrimin cream for yeast, but a urologist is often needed.
