I need some help in diagnosis

Hello all,

I am 25, I have more than a few health issues and bad sleep is probably one of the biggest of all. From the past 3-4 years I have been suffering from sleep issues.

Before 1-2 years, whenever someone wakes me up, I used to involuntarily scream or jump or do moves with my hands/legs and people used to get scared. (I am not sure if I still have this issue now, as no one wakes me up now)

Currently, in the morning, most of the times, I wake up at some 5 AM or 6 AM (I intend to sleep till 8 AM, but I wake up automatically at 5-6 AM). I still feel very sleepy and tired, but if I sleep again, that new phase of sleep is like hell, I get lots and lots of dreams, I get dreams of things I do daily, I feel that I woke up and am checking my phone but then I am still sleeping.. After this phase of sleep, I feel very tired, my eyes sometimes paining and... if I wake up and then just close my eyes even for just a minute, I keep new random dreams coming.

Even during day this continues. Depending on how drowsy I feel, whenever I close my eyes (any time of day), I start getting dreams within 2-3 mins or sometimes instantly. Not sure if these are dreams, but I feel like I am bombarded with some random dreams as soon as I close my eyes.

I sometimes feel confused if I did something in a dream or in reality.

My memory is absolutely pathetic, I cannot focus on anything, I keep forgetting what I am doing and do something else.

It feels like I am in the middle of sleep whole day.

And I am tired all the time, the only solution I find is, to sleep. After sleep, for 2-3 hours, I am better and then start feeling tired again. But sometimes if the sleep is bad, I feel even more tired. Whenever I work continuously for 2-3 hours, I feel very tired and most importantly drowsy.
