If you don't want to get pregnant, you need to use protection from beginning to end. He can ejaculate with a condom on and the only thing that would hold him back is his own mind, so he can get over it. Next time, don't let him pressure you into having unprotected sex and make sure your boundaries are very clear about you are and are not willing to do.

It doesn't matter if he ejaculates. Pre-ejaculation is ALWAYS present in a male when he's aroused. Yes, you should worry. It doesn't matter if it's for 1 second, or 1 hour. You NEVER have sex without condoms unless trying to conceive. It's utter BS if he says he can't ejaculate with a condom on. He'll get over it, or he'll become a dad real soon.

Because while pre-ejaculation contains sperm, it's not a hugely large amount. The fact that he had just ejaculated means sperm is right there in the urethra and it gets in your almost immediately.

It would be your choice. I'm curious why you took the condom off though? If you don't want to be pregnant, that's a pretty stupid move whether he ejaculates or not. Plan B is $50 in most areas, and if you're under 17 you need a prescription to get it. I doubt you're pregnant, but in the future be more responsible and use a condom from start to finish. Also, if you choose to have sex again after that, you again need to use a condom. Ejaculation would still be in the urethra and puts you at even higher risk of pregnancy than it would before.
