Thyroid Advice, actively seeing doctors but can't seem to get answers

TSH Level 196 and ultrasound scan read this "The right lobe measures 2.1 x 0.6 x 0.7 cm. The isthmus measures 9 mm. The left lobe measures 1.8 x 0.6 x 0.8 cm. The gland is heterogeneous. There are no thyroid nodules."     Any ideas?  Tired, chest pain, feel like my muscles ache, can't think clearly or remember anything.  Have struggled with hypothyroidism for 13 years.

On the hematocrit result, the only thing I know is what I found as follows:

May 26, 2016 - The definition of normal red-blood cell percentage also varies from one medical practice to another. Generally, a normal range is considered to be: For men, 38.8 to 50 percent. For women, 34.9 to 44.5 percent.

Please double check the FT4 and FT3 results.  With you taking 200+ mcg of Levo, I would expect the FT4 to be high, not the FT3.

Is TSH the only thyroid related test being done for you?

They did a Free T4 was .29 and Free T3 was 2.27.  Tests were ran on 12/27/16.  Normally on 200mcg of levothyroxine and they increased to 250 but was referred yesterday for the ultrasound as well
Hematocrit came back high at 48% the lab results say as well

I should also mention doctors have me on 250mcg of Levothyroxine
