Can anyone help me to interpret this readings.

Right lobe of the thyroid is enlarged due to the presence of 4.5x2.5cm fluid-filled mass. Left lobe of the thyroid is in normal of size and echotexture.Left lobe measures 4.3x 1.1cm. No calcification was appreciated. Absence of cervical lymphadenopathy. Visualize vessels are normal in caliber. Remainder is unremarkable.

IMPRESSIONS: Cystic thyroid nodule, right.

is the size of the right lobe still in normal condition? how about the "impressions" on the readings?

The right lobe is not normal because it's enlarged due to the nodule; however, the nodule is nothing to worry about at this time.

The impression simply states that there is a cystic nodule on the right lobe.  It does not suggest any immediate follow-up, however, it's always wise to discuss findings with a doctor and make sure repeat ultrasounds are done at the appropriate times.

It would also be good to have thyroid hormone testing performed to determine if the patient has a thyroid condition, such as hypo or hyperthyroidism since nodules are common with both.  It would also be prudent to have antibody testing done to determine if the patient has Hashimoto's or Graves Disease, which are both autoimmune thyroid conditions. Nodules are common with both of these as well and both autoimmune conditions cause either hypo or hyper status.

A fluid-filled mass (cyst) is, typically, not anything to worry about.  It's the solid masses we have to be concerned about.  There doesn't seem to be anything else about your ultrasound that's of concern.

Are you having symptoms of a thyroid condition?  If so, what symptoms?
Have you had any thyroid blood test done, such as Free T4, Free T3, and TSH or thyroid antibody tests to determine hyper or hypothyroidism of if you have an autoimmune thyroid condition?  If you've had thyroid blood work done, it would be helpful if you could post the results and reference ranges, so we can determine your actual thyroid hormone status.
