Your labs are very close to hyperthyroidism (they are high in the ranges), and with your symptoms and family history, I think you can assume you are hypER, not hypo.
You not only need antibody testing, you need to question your doctor as to why he's giving you thyroid hormones when your thyroid hormones are already (too) high.
Okay i will ask him to check antibodies, thank you. At first when i noticed that i have swelling of the neck and with my symptoms i thought i was hyper, because my twin sister has hyperthyroidism, and when i got my lab results the doctor said its normal.
My constipation started early this year only, it came back normal. But now i'm experiencing it again i'm using stool softener when needed.
My bruises are like the typical ones but sometimes it has little red/purple dots on it. I only got bruises on my right knee.
You should ask your doctor to check antibodies. TPOab (thyroid peroxidase antibodies) and TGab (thyroglobulin antibodies) are the two markers for Hashi's. Hashi's is typically hypo, but can be characterized by hyper episodes in early stages. TSI (thyroid stimulating immunoglobulin) is the definitive test for Graves' Disease (hyper).
Except for the constipation, your symptoms sound like you are hyper. (Is constipation a lifelong condition?) So, I think it's very important to question your doctor and ask him why he's giving you meds that could make you even more hyper. Doctors do make mistakes at times. You have a right to know what his treatment goals are.
Question on your bruising: Are your bruises the typical diffuse bleeding under the skin you get when you bang into something, or are they smaller, darker and more concentrated? Are they painful?

Yes i have insomnia, i feel my heart beats fast, and I lose weight, im on my 5th day taking levo, and i don't have chance to check blood pressure.

Do you have insomnia (can't or don't need to sleep much) or do you initially fall asleep, but have trouble staying asleep?
Is your breathing difficulty due to your swollen thyroid pressing on your windpipe?
Has your heart rate increase or decreased? What about your blood pressure? Any weight gain or loss?
I bruise for no good reason, too, but I haven't been able to link it to thyroid in any way.
Taking levo should not make you sleepy. All the different tablets have different fillers (inactive ingredients). Some people are sensitive to the fillers. How long have you been on levo?
Has your doctor done any antibody testing?

Your labs are, indeed, at the "outer limits of normal", but they are very much on the hypER side of normal, especially your FT4. Your ultrasound indicates that your thyroid is enlarged (goiter). Sometimes levothyroxine is given in an attempt to shrink the thyroid by letting it "rest". Perhaps that's what your doctor is trying to do???
I would certainly question your doctor and ask what he is trying to accomplish by giving you levo. As far as your labs go, you have more than enough FT4 in your blood already (levothyroxine is T4). He may have a valid reason for his choice of meds, but I definitely think it merits an explanation at the very least.
It does take 4-6 weeks for a dose to reach its potential in your blood, but the vast majority has stabilized in 4 weeks. Retesting in 30 days is fairly typical when meds are being adjusted. Yes, you will have to have the blood work done again.
