Sounds like an immune system blockage.. could be liver issues. Any enzyme tests?
An Rt3 test could fall into the mix here to get as G says too.
Have you done a basal body temp test tracking?
My thought as something you could try would be to flush out the liver some and see if that releases the T4 hormone. You could be falsely high on the T4 lab due to the liver fighting the conversion with some issues and once you clean that out - purify - then the real conversion would take place.
You could get a rush of T3 hormone temporarily due to that realease but then your labs would come back more true to your real levels and be able to pin point a treatment dosage better with that liver being cleaned out again.
KEVIN.... Hi! You are NOT converting well if your Free T3 is at the low normal range. I don't know why your doctor is saying this. You are getting more than plenty of T4 but not enough T3.....your TSH should look supressed when on a combo med. Are you still on Armour???? I don't think it is working well for you. That is why I had to change. My numbers were doing weird things and I kept getting sick.
Please reconsider what you are taking and look at your choices....I know....I have been there.
Your numbers should not look like this if you are on meds that are working. ALso....you are not suppose to take your meds before your labwork...so you did the right thing.

If I were you, I would almost be tempted to test FT3 BOTH before and after taking meds. It might be a really interesting comparison.
Had you taken your meds before the last test when your FT4 was over range and your FT3 was in the upper third?
I think FT3 is "more" important "most" of the time. You mention "higher dose". Were your meds increased?
Most doctors care more about FT4 than they do FT3 to the extent that many of them don't test FT3, believing that if FT4 is fine, so is FT3. Both have to be adjusted properly to feel well. If your meds are properly balanced, FT3 should be higher in its range than FT4 is in its. Your profile is a bit "upside down", but it all depends on how you feel to a very large degree.
Don't give up...go by how you feel. We're all different. It takes patience, patience and more patience...
TT3 is considered an obsolete test and of limited usefulness. It tells the total T3 in your blood, whereas FT3 tells what's actually available to your cells.
IBS could be the cause, but low FT3 could also cause the IBS. Chicken and egg syndrome...it depends to a large extent on the FT4/FT3 balance.

i didn't take the dose that day so i think that might be part of it, last test i was in the upper third on the free t3 but over range on free t4??? i had the rt3 in the past was fine not sure how much the free t4 really matters as long as the free t3 comes up which it will on the higher dose?/ do the doctors care about the free t4??? not sure about the ratio on the compunded??/ last test 6 week ago i was 320 range 230-420 on the free t3? i don't know ?? Hmmm... poor gut health? I have that problem because of IBS. I wonder if that is the cause of my low range TT3. Thanks for the heads up.
Converting fine? When you're converting "fine" your FT3 is higher in its range than FT4 is in its. Considering that your're on meds with a high T3 content, it looks to me like you're hardly converting at all, or something else is awry, like you're RT3 dominant. Have you ever had an RT3 test?
What's the T3 and T4 content of your compounded?
Any number of things can affect conversion...liver problems (since most conversion takes place in the liver, but it also happens at numerous other sites throughout the body), selenium deficiancy (the enzyme that converts T4 to T3 is selenium-based), metabolic issues (poor gut health),etc.
When my thyroid gland becomes inflamed my FT4 goes up and my FT3 or TT3 goes down. I've seen that twice now in my numbers. Feels like I've got the flu or a cold coming on but the feeling only lasts a few hours goes away and comes back, etc.
Right now this is happening quite often.
Looks as if your hypo again as my numbers would show too.
