Thank you for taking the time to help me. I have been taking d3 for years and b vitamins for about a year. My b12 is at 461, vit d is at 46. I will up my intake of those. I have no labs on ferritin. I am seeing her this friday and will ask for meds. Thank you again!

With those symptoms, your Free T4 so low in the range, and the relatively low TSH, I'd say that you have central hypothyroidism. When your get a Free T3 test I am sure it will also be in the lower half of its range, which is also consistent with being hypothyroid for many people. Central hypothyroidism is caused by dysfunction in the hypothalamus/pituitary system that results in relatively low TSH insufficient to adequately stimulate the thyroid gland. Central hypothyroidism is overlooked quite often because of the reliance on TSH as a diagnostic.
A good thyroid doctor will treat a hypo patient clinically by testing and adjusting Free T3 and Free T4 as necessary to relieve symptoms, without being constrained by resultant TSH results. Symptom relief should be all important, not just test results, and especially not just TSH results. You can get some good insight from this link written by a good thyroid doctor.
So you need to discuss all this with your doctor and see if he is willing to start you on thyroid medication. You also need to find out if your doctor is willing to always test you for Free T4 and Free T3 each time you go in for tests, and also treat you clinically, as described above. If not, then you need to find a good thyroid doctor that will do so.
Also, since hypo patients are frequently too low in the ranges for Vitamin D, B12 and ferritin, you need to test those and supplement as necessary to get them up to optimal levels. D should be about 60, B12 in the upper end of its range, and ferritin should be about 60 min. for women.
I have gained 50lbs in two years even dieting and working out some, hair loss, skin rashes, headaches, heart palps, and hot flashes are terrible but could be my age. I am 47 and although my hormones are low. I am swollen and tired a lot. I am very moody and a bit depressed. The headaches are migraines at least once a month and this past month I had 3 migraines, they start off with this kaleidoscope like vision.
Many doctors would tell you that those labs are normal and no treatment is necessary. In reality, your Free T4 is too low in the range for many people. It is unfortunate that you were not tested for Free T3 also, since Free T3 correlates best with symptoms, while Free T4 and TSH do not correlate. You should make sure that Free T4 and Free T3 are both tested in the future.
