"TSH, 3rd generation w/reflex to FT4" is an instruction to the lab. It means: test TSH, and if it's not within reference range, then test FT4 also. So, your doctor isn't testing anything but TSH. FT3 and FT4 are the actual thyroid hormones and much more important diagnostics than TSH. I think it's an excellent idea that you are getting a second opinion. Make sure this new doctor runs FT3 and FT4 and treats based on them rather than TSH.

TSH is not always reliable, as it is a pituitary, not a thyroid hormone. FT3/FT4 tell us exactly what the thyroid is doing.
