Not much Info

Ok, For about the last 2 months and a half I have been moody, depressed, forgetful, extremely exhausted and weak. I thought that at first it was my menstrul cycle so i went and had my gyno put me on yaz for PMDD. Well it seemed to help a little but then my memory for things started getting worse and i couldnt remeber to take my pill. So i figured at my yearly pap which was due in march of this year, i would ask the doc to switch me to depo shot. Which she did. But during my exam she felt that my left thyroid was swollen and being my age she was curious as to why. i am 32 on may 2nd. So she ordered tsh and antibodies labs. well my tsh came back 2.79 which is in the normal range but i came back positive for antibodies- glubolin was 80 and peroxides were 1470. So she had me follow up with my PCP. He then sent me to the hospital for an ultrasound and the ultrasound came back with a nodule on the right(not sure exactly where) and i actually just found out the size today(3.9x1.6x1.8cm). He then sent me back to the hospital for an uptake and nuclear thyroid scan...(i havent gotten those results yet) but i noticed on the screen of the uptake that the count was 7935 for my thyroid and 101 for the scan done over my knee for the bloodstream. Can anyone tell me what this means? i only know the normal range is 10-30% and well i didnt see any %'s on the screen. I am so nervous with all this and waiting for the results, as my mother has hypothyroidism and has a lump that has not been investigated but she has been presenting trouble breathing and continuous cough for 32+ days now....and i know that can be a sign of thyroid cancer and that it can be hereditary. She is talking to her doc today about investigating it.

I agree regarding seeing an Endo.
They may not perscribe medication, but they may run more tests that lead to starting medication.

one more question-im also suffering from skin colored acne and regular looking acne all over my face....mostly my forehead and down near my jawline into where my ear meets down into my neck. the skin colored ance almost looks like tiny hives, sometimes they itch, sometimes they hurt and sometimes they are both. any ideas if this is related to my thyroid?

I would definitely go see an endo.  They started me on synthroid with a T4 of .74 (T3 and T3 uptake were low too) and TSH of 2.63 - what the endo called "subclinical hypothyroidism."  I have a post too with similar numbers to yours.  As of last week my T4 is up to like .94 or something and my TSH is up to 3.23 (still before treatment).

I never gained any weight.. but I can't lose it for the life of me.  No amount of running seems to take any weight off.. so thank God I'm not overweight!

Seriously - go see an Endo.. my primary care physician completely ignored the results of the bloodwork even though the lab flagged T4, T3, and T3 uptake as low, just because TSH was deemed normal.  The endo says anything above 2 and below 5 is considered subclinical hypothyroidism (considering thyroid hormones are low).

ok-now its been 3 months and no repeat scan for the goiter but i did get my levels done again. recap of last labs:

okay so now im really confused. i have gained 8 pounds in 3 months and im not prego according to 2 home prego tests. i have been bouncing between every mood you can possibly think of including depression. i am also under a lot of stress with my children.(long story-one has a disability)
my doc says that all the labs are within normal range and stable-which makes no sense because my tsh has obviously gone up and my globulin as well.
but he still says nothing about hashimoto"s.
i have contacted my insurance and they have told me that my gyno can refer me to an endo.
so i am going to see her next week to determine if i am prego or not for sure and if not i am requesting she refer me to an endo. because even if it's not my thyroid making me gain weight-something is. you just don't gain that much weight from a slowing metabolism in that little time.
if anyone would like to comment on my results and what i should do, or if you think i should do what i am planning to do......please do so....the more info i have going in the better.
update on my mom-the lump was removed and biopsied-result: hashimotos

I am out of my element when it comes to discussing nodules. I know just enough to be dangerous.

Cold nodules are viewed as suspicious for cancer and if they are found an FNA is recommended. There were no cold nodules found.

Hot nodules are producing extra hormones you do not require. There were no hot nodules found.

Your nodules appear to be benign, which is good, if you have to have nodules.

I have no idea what an uptake of 22% means, whether it is good or bad.

Someone better informed will be able to answer your questions.

okay i just got the actual reports of ultrasound and uptake/scan.
ultrasound reads as follows: Indictaion=enlarged left lobe thyroid
Ultrasound examintion of the thyroid is performed and interpreted w.o previous for comparison. the left lobe is normal in size, measuring 2.7 x 1 x 1cm. the right lobe is mildly enlarged, measuring 3.9 x 1.6 x 1.8cm. Echotexture is mildly heterogeneous bilaterally. there is suggestion of the small dicrete isoechoic nodule posteriorly in the mid to lower pole on the right measuing 6mm in greatest diameter. No other discrete nodules are seen. No extra throidal mass is seen, nor is ther evidence of adenopathy.
impression: Heterogeneous gland w/rgt lobe enlargement suggests multinodular goiter. Lab correlation is recommended. radionuclide scan could be considered for further evaluation, if indicated.

Thyroid uptake and scan
indication: thyroid nodule
16 uCi I-131 in the sodium iodide deformed administered 4-16-08, folowed by 10.3 mCi Tc 99m sodium pertechnetate administered intravenously 4-17-08
Examination demonstrates more prominent uptake in the rgt gland and the lft. No appreciable cold nodule seen. the calculated uptake is 22% at 25.4 hrs, within normal limits.
impression: NO appreciable cold nodule identified.    22% uptake at 25.4hrs within normal limits.


What does this mean? does it mean i may have cold nodules but not suspicious ones? does it mean i have no cold nodules? does it mean my nodules are warm/hot or they just don't show anything? is that possible?
any opinion is appreciated. as i still havent made up my mind whether to get a 2nd opinion from an endo.

You can have both of those types of antibodies and not be diagnosed with Hashimoto's, but you will eventually.

A diagnosis of Hashimoto's must meet certain criteria.

Specific antibodies, and either a goiter, or bloodwork showing that you are hypothyroid.

Your TPOab is 1470. That is high enough to assume that damage is occurring. TPOab is not specific to Hashimoto's. It is an indicator of one of several auto-immune diseases. When it is found in high titers, or numbers, it usually points to thyroid disease.

Your TGab is 80. TGab is an antibody that IS specific to Hashimoto's, and when you find both antibodies present in high titers, it's Hashimoto's. Your TGab is not excedingly high, but it is high enough.

You have a goiter, caused by high antibody titers.
You have Hashimoto's.

Your TSH is a little high, and your Free T4 is a little low.
You would benefit from treatment now. Whether or not an Endo would agree and start you on meds depends on who you see.

It would be worth seeing an Endo to review the scans, talk about your multi-nodular goiter, and your blood levels. Starting hormone replacement meds may help a little with the goiter, which may be reason enough to start you on meds.

The Endo may also have a different opinion regarding the scan of your nodules.

my primary talked to me yesterday and said the nuclear scan was normal and that unless i had symptoms we would redo my tests in 3 months. i talked with my gyno today about what the doc said and she said that it was reasonable, but if i wanted a second opinion i had that right and an endo would maybe look at it differently. Any suggestions?

you said my antibody tests show me i have two kinds of antibodies for Hashimoto's, so here's a question, can you have those two antibodies and not have hashimoto's? my doc hasnt said anything to me about having anything other than a multi nodule goiter. i had a full metabolic work up done in feb due to kidney stones and all those came back normal. and according to my doc my tsh is normal and my nuclear scan is normal. has anyone had these type of results?

the prob is i dont have that info yet-so far as i know my tsh and the antibodies were the only blood tests i got.....i have to call and find out....i plan to get a copy of the reports for myself.
my nuclear scan----according to another doc in the office was normal-i still don't know what this means other than not having cancer. she couldnt tell me much at all. my doc is supposed to call me tomorrow to talk with me about it and what the next step is. I apparently don't have a nodule, i have a multi-nodule goiter. which still doesnt tell me much. if anyone can help me out with that, it would be appreciated.

Prepare for the best though - You are at the beginning of some Dx - so the more you worry - the worse you could be.

MAny here are very familiar with your situation and can give you a world of info - Post your Free T's if possible.

Your TPO's is high so those would be great to see.

The only thing I can tell you about your thyroid uptake scan is, it worked. What it SHOWS is up to some technician to figure out.

I imagine they are looking at two thing with the scan. Does your thyroid uptake? It did. Do you have any hot nodules? Don't know.

I am blissfully ignorant about nodules, but it sounds like that one of yours is fairly large. They want to know if it or any others are hot or cold.

Your antibody tests show you have two kinds of antibodies for Hashimoto's, which is an auto-immune disease.

Hashimoto's is the most common form of Hypothyroidism, so you are in the company of millions in that regard. Auto-immune conditions stink, because one tends to collect them like baseball cards.

It is not good news, but there are millions upon millions of us.

The nodule is what is being assessed right now. Trying to determine if it is producing hormones or if it is a risk for cancer or if it is just hanging out.

Your TSH is a little high, but not much. There is no mention of Free T4 or Free T3, your thyroid hormones, but they are probably just a little low, but within lab limits.

Your swollen lobe is being attributed to thyroiditis until they finish running tests. Then it may become Hashimoto's thyroiditis, or that may wait until your hormones and TSH come back showing a more extreme deviation from "normal".

I hope this isn't making you more nervous. I'm just trying to explain what the thought processes are at the doctor's office, and what the labs you presented mean.
