TSH is a very poor indicator of thyroid status. Her TSH is nowhere near hyperthyroid level. It is most likely indicative of central hypothyroidism, which is a dysfunction of the hypothalamus/pituitary system resulting in relatively low levels of TSH that do not adequately stimulate the thyroid gland to produce hormone.
Yes, your friend needs to test for Free T4 and Free T3, since those are the biologically active thyroid hormones. In addition she should test for cortisol, Vitamin D, B12 and ferritin. If she can get those done and post results and reference ranges shown on the lab report we will be happy to help interpret and advise further.
In diagnosing for hypothyroidism, even more important than test results are symptoms. Please tell us about any symptoms she has.
