Thyroid, Liver, Migraines-Pediatric

What test should a 11 year old boy have done next (just want to be on top of stuff), So far to date my son has had  a goiter for 2 years that has been felt with hand by DR. with normal thyroid blood work according to the Dr.'s, Hypothyroidism runs in the family, I have it and have other family members with it, my husbands mother has it, so it runs on both sides for my son. Recently I requested to have an ultrasound done on his thyroid-just for my peace of mind, we go back this Wednesday for actual results, but my son's case manager for his DR. called me and moved his appointment up a month and said he had a probable cyst on his thyroid. I just want to know what should or is normally next to make sure all is good, or will the ultrasound show what we need to know exactly?! Feel like I get the run around with him so much on his thyroid, one Dr. said thyroid problems in boys just don't happen often, so it's nothing to worry about, one said his thyroid didn't seem that enlarged to worry and I decided to go back to the one that was out of state that diagnosed him with the goiter to get things figured out. He has been having high liver enzymes for about a year and one of the Dr's that we were seeing in our state now found the liver enzymes high on him, but the nurse called me after an ultrasound showed nothing on the liver and said well that may just be the normal for him, they were highly elevated. Well I expressed my concern to the Dr. that we just started seeing again and she said it's absolutely not a normal for anyone to have that high and ultrasound said appears to be fatty liver. Other than that, he experiences severe migraines. Please help guide me on what to request from the Dr. if they just decide that we wait it out for another year!

Could be related, I have read the links between liver and thyroid, but when your thyroid looks good in blood work it's hard to tell a lot of dr's something is wrong!!  When they did the blood work for antibodies, it came back saying that he was fine in that region. All blood work says his thyroid is great. He has so many hypo. symptoms, low body temp usually around 96.5, joints hurt, just has no desire for anything. His vitamin D level was a 13.  We met with the Dr. yesterday, turns out ultrasound showed something wrong with his kidneys now. Anyway, we are headed to a gastro dr. and an endocrinologist, getting a ct scan done and some biopsies of liver/kidney and thyroid. For once I feel like they will find the answer, just hope it comes soon with good results and before something else is affected. Thanks for the reply!!

I had elevated liver enzymes for years due Hashimoto's thyroiditis. When I started thyroxine, my liver enzymes dropped back to normal.  Hashimoto's thyroiditis runs in families due to genetics; the genes particularly mentioned are HLA DR3, HLA DR4, HLA DR5.

Thyroid peroxidase antibodies (TPOAb) are found in 90 - 95% of cases of Hashimoto's thyroiditis. Thyroglobulin antibodies (TgAb) are found in 55-90% of cases of Hashimoto's thyroiditis.
