It is normal for RAI patients to go on anti-thyroid meds afterwards if Hyperthyroidism was the reason for the RAI.
The thyroid doesnt abalte straight away and can take some months but just before it does die off....it dumps a huge amount of thyroid hormone which can cause hyperthyroidism and therefore beta blockers and anti-thyroid meds are wise until your wife goes hypo.
I was hypo after 5 weeks so watch for the signs.
I did have a dumping effect and it lasted approx 3 days.
In those 3 days, I took beta blockers.
Dont let your wifes TSH go over 6.0 if possible then she wont crash hypo too hard.

I was told to resume beta blockers and methamazole (similar to tapazole) 48 hrs. after RAI.
There is a good chance that she may go hyper from the thyroid dieing off and dumping more hormones into her system. The RAI can cause temporary thyroiditis.
When in doubt, check the handout sheet you got from the Dr.
For what thyroid condition was the RAI done?
Are you sure the doctor wants her to start taking tapazole and not one of the thyroid hormone replacement meds like Synthroid, Levoxyl or generic levothyroxine prescribed for hypOthyroidism?
