tsh at 4.90

if your tsh at 4.90 range 0.35 - 5.00 do you treat?  I currently take thyroid med. 30mg once per day.  I take the dose in the morning, it gives me diarrhea and insomnia at night.  I tried Synthroid lowest dose .00125, had to discontinued because of inability to walk since felt the med. was too strong and super hungry all the time, it puts my system in overdrive.  it is such a small dose, there is no way I can take higher dose.  should I be looking at something else to correct my tsh?  as taking thyroid medication does not seem to agree with me.  It is also in the normal range t3 & t4 are in the range.

I'm sorry, your posts are very disjointed and I'm not sure I understand them... first you say you were taking ERFA, then you you're taking Armour... I know they work the same way, but they aren't the same medication.

You should talk to your doctor about stopping the medication for about 3 days, then starting back a lower dose. If you got terrible diarrhea on only 15 mg of ERFA, that tells me you might not need the T3 and could possibly be better off on a T4 only medication.  That's something else you should talk to your doctor about.

Your symptoms are those of over medication... When you added the T4, did you decrease the ERFA?  If not, you really over medicated yourself...

What dosage of ERFA were you taking and what dosage of T4 were you taking?  How long did you take the T4?

Was the TSH of 4.90 before starting med or after you'd been on it?

You can't just stop taking medication; you may feel better for a while, because your FT3 was too high right now, but you will go hypo again very soon, because ERFA has a lot of T3, but it doesn't stay in your system very long.

What thyroid medication are you taking?  Is it Armour or NatureThroid.  Certainly appears to be a desiccated med of some type...

Rule of thumb (where most of us feel best) is to have FT4 at around mid range (50%); yours is only at 35% of its range.  Rule of thumb for FT3 is upper half to upper third of its range; yours is at 81% of its range, which is somewhat higher than you probably really need it.

Increasing your med will most likely drive your FT3 higher, but your FT4 will continue to lab behind... It's most likely the low FT4 that's causing issues.

Many people on desiccated hormones have reduce the dosage of desiccated medication and add a source of T4 only medication in order to get a proper balance between FT4 and FT3 in order to feel right.  But again, TSH is not what you should be worrying about... symptoms come first, then FT levels...

What, if any, symptoms do you  have?

free t3 is 5.1, range 2.6 - 5.7 pmol/L
free t4 14.8, range 12.0 - 22.0 pmol/L

What's the name of the thyroid medication you're taking?  Is it a desiccated hormone?

Unfortunately, TSH is a pituitary hormone, not a thyroid hormone and there are a lot of things that affect it, other than thyroid hormones. You shouldn't worry about normalizing TSH; you should worry about whether or not your actual thyroid hormone levels, Free T3 and Free T4 are where you need them. What are your actual Free T3 and Free T4 levels? Be sure to include reference ranges, as those vary lab to lab and have to come from your own lab report.

Is the 4.90 while you are on meds? TSH is such a 'personal' number. Many people feel good at higher levels while someone else at that level may feel like death. The important thing is how you feel, not the number.

What are your other levels...ferretin, vitamin D, B-12, DHEA? if those are out of wack, you'll def feel it. Also, is your dr monitoring your cortisol as well?
