For some reason, many posters don't bother to respond to replies,
specially when they realize the "silver bullet" they were expecting
was not there.
Unfortunately, a lot of conditions presented here, are rather complex,
time-consuming and not addressed adequalely by a medical system
which is over-burdened by financial, time and political contraints.
Your case too, may fall in under this.
When a condition persists after many years, it becomes questionable, wether the current diagnostic/treatment appoach is leading to anything useful.
I'm glad your state of mind seems to be ok and you remain hopeful,
but, my concern for you is the long-term outlook.
Chronically interrupted sleep patterns, affect hormonal levels, which are associated with weakened immunity, accelerating aging, depression, cfs, cognitive issues and more. You body cannot sustain this indefinitely.
I urge you to read again my original reply and answer the questions as best as you can.
Leaving this up to your doctor, is almost like leaving it to chance, judging from the results so far.
Taking charge and being more proactive will serve you greatly.
You're still very young and have a much better chance for recovery before
any significant damage takes place, down the road.
Forgive me for speaking frankly, and for alerting you, but, I understand
the seriousness of your condition as I have gone something similar,
BPH (benign prostate hyperplasia)- affecting males only- but with almost identical symptoms.
Luckily, I pursued it mainly on my own and successfully self-treat with herbals.

Thank you so much for your reply, I do struggle after a bad night and tend to go down hill around 2pm,
I am female and 28 years old , my symptoms are both day and night but they do seem to be worse at night , I do not have any pain but after a bad bout I can have an ache that feels like I have strained my muscles. I have had the urodynamics test which showed That I have very slow urine flow but the volume wasn't to bad , the camera test I had not so long ago showed all was looking good inside the bladder and urethra , I have also had scans on my ovaries that where all clear and no signs if utis , I have not had a complete hormonal panel ? Or any neurological or physical test , but maybe This will be the next step for my doctor , I really hope to find the answer soon and appreciate your help .

How can you possibly function the following day? And being chronic, this must have affected your endocrine/hormonal system greatly.
Some questions:
Male or female? I see your profile says female . Pls verify.
Age?
Have you had a complete hormonal panel? Estrogen dominance ruled out?
Is it just urgency or is it actual ?
Is it painful when urinating?
Is emptying an issue?
Have UTIs & Interstitial cystitis been 100% ruled out?
Have neurological causes been checked? S2 -S4 peripheral nerve dysfunction?
Have psychological causes been explored?
Would you or could you estimate or measure day and night urine volume?
This is a significant clue , to my opinion.
