Help with symptoms?

I am a 29 y/o caucasian male.   I haven't been feeling well for about 5 years now with little help from doctors.  

I was diagnosed as hypothyroid 2 years ago with a TSH of 4.51 with in range T3 and T4; I was placed on 50 mcg Synthroid and have been on that ever since with little improvement.  My TSH is now 1.8 and also take cytomel.

3 months ago my family doctor started me on testosterone injections to see if it would help with the fatigue, it did help somewhat, especially for recovery after workouts.   My initial levels were in range but only by a few points, it was very low normal.  

I also take the antidepressant Pristiq which doesn't seem to help with any of symptoms, I told my doctor that feeling bad everyday depresses me so he started me on an AD to make sure it wasn't depression causing the symptoms.  

As of now, my thyroid and testosterone levels are in check but I still have the following debilitating symptoms:

Fatigue, body ache, itchiness, occasional kidney pain, hives on my arm (raised red fleshy bumps), itchy anus, flushing, constipation, occasional dizziness/fogginess, weight gain and inability to lose weight.

I've noticed when I have caffeine my symptoms sometimes get worse but sometimes get better.  

Today I had total body pain, maybe my body going through withdrawals from the caffeine I thought.  I had some caffeine and the pain immediately subsided and I got my energy back but was replaced with itchiness, flushing, kidney pain, and anxiety.  

Is this a kidney problem or an allergy to caffeine?  I can't seem to get through the withdrawals because of the magnitude of the pain.  

Please help!

I know, do I know where you are coming from.  Doctors DO have a tendency not to take as much time with you if you are taking medications for depression or the like for a mental health disorder.    If you have a mental health disorder you are often too complex for them or they simply don't know how to treat you especially with some of the issues you mention.    It happens to me so often, I can't count.  Don't ever give up though, OK!!!! I'm still trying to find the right doctors to help me and it's been years now.

Dude, I used to have the same problems.  I found out my diet was totally realted to all this symptons.  I have hair loss, urinary problems, chronic fatigue syndrome, headaches, lots of constipation (looks more like allergies cause it was pretty much all of the time, like a constant flu), i felt sometimes like very week legs and articulation pain.  I did not sleep good and my anxiety levels were off the roof... I went to a lot of doctors, and i had to run like a million tests and nothing...someone told me about how this is totally related to diet and gave me a book called The China Study... after that I tried a raw vegan diet for a month and i swear I fell in heave, with tons of energy, I slept like 7 hours a day when I used to sleep 10 to 12... my hair stoped falling, my skin looked awesome, pain was gone and anxiety too.. constipation was gone and i felt damn happy like never.  Check it out, maybe you have the same problem as me.  Since I know this diet I've been fighting with temptation 'cause the hard thing about this is doing the diet... =S it's tought but we can do it!! I'm commited to doing it.  =) Don't forget to exercise, drink lots of water and sleep good enough... that also helps.

Get checked for Celiac disease and nutritional deficits like vitamin D and B12.  

Studies show that with thyroid problems, doctors need to add more supplementation if the patient still feels bad even when the numbers are "right."  Adding just a little more supplement will usually correct the symptoms.  If your regular doc won't do it, find another.
