Burning penis during and after sex

I am a 58 yr old diabetic. In the past 6 mos. when my wife and I have sex, my penis gets very irriated on the outside just under the glans penis during and after sex and when I ejaculate it really burns when the semen touches the outside of the penile shaft. When I remove my penis, I cannot hardly touch it to treat it. I apply A&D ointment for a few days and it clears up until I have sex again. I have never had anything as painful as this. I just went to a urologist and he said he did not see any problems and just to apply the A&D ointment but I cannot stand this much longer. It has reached the point that I do not want to have sex because it hurts so bad. I have not had sex with anyone other than my wife and I am sure it is the same with her. I am not on any new meds. Any suggestions at all? I am at my wits end.

Yes, we have tried water soluble lubricants with no success.

My first question is "Are you using a water soluble lubricant when you have sexual intercourse?" Women who have passed menopause do not lubricate their vaginas as they did when they were younger (and I am assuming that your wife has gone through menopause)/. Thus, there may be too much friction. Vaseline (an old favorite) is not that slippery and the cleanup is messy. A surgical lubricant such as K-Y Jelly/Surgilube works well as do the  lubricants sold expressly for sexual purposes (Astroglide is the first one that comes to mind, but their are others). Apply some to your penis before entering your partner and put some in her vagina and around the outside. Is your wife douching? You may be having a problem with the chemical that she may be using. If she feels that she must ******, ask her to rinse her vagina with plain water  afterwards. If this is not effective, see a dermatologist while the irritation is fresh.
