penis/foreskin problem

hello

i am 22 year old male, in good shape with problems about 3 years now. my penis is small, about 4 inches erect and very small when
flacid(1 inch) and foreskin does not always cover the entire penis.

About 3 yrs about my penis started to become very dry. with the small size when it is flacid, the forskin became tight and actully tore the penis, leaving a scar and a white spot.

i visited a dermatologist, who gave me cream, cant remember the first one, vitamin e,c and k and elidel, and e45 cream. different thing where tried for six visits for about a year with very little favourable results.

the white spots and scar was there still and futher developed black spots and i visited another dermatologist. he said there is nothing wrong and it is variation an recommended vaseline for dryness and soreness

it has been more than a year now and there more so much brown/dark/white spots on my penis head and very wrinkled. it looks disgusting. the underside of foreskin is wrinkled when retraced and also has changed color from pinkish to dark brown spots.

i cannot wear tight underwear as with the small size when flacid will cause the foreskin to wrap around the head and make marks. and i cannot excerise without rubbin alot more vaseline. my penis would retract when i excerise,an the foreskin will cause marks.

When my penis is full when flaccid  the foreskin does not affect the penis head an i dont have need for cream. it is only when the penis retraced the foreskin covers the penis head 'crushes' it leaving marks, which become more and more permanent. some of the marks resemble lines on the hips made by jeans.

When my penis is full when flacid the forskin is relaxed, the marks dont show as much. and there are no marks caused by the foreskin. is there any way to keep my penis full when flaccid?

i use vaseline petroleum jelly, an i am worried about how to keep it clean, it does not wash off easily when i bathe.

the black/brown,white discoloration of penis head, with wrinkles  and marks along with my size makes me very uneasy and not confident.

please advise, should i visit a urologist/ another derm or leave it as is? also what other cream can u suggest.

Obtaining a second opinion with a dermatologist or urologist should always be considered.  It is difficult to give concrete recommendations without seein the lesion in question.

Testing for possible fungal infection (i.e. with a swab for culture) can be considered, since this can lead to discoloration.  

If an fungal infection is present, you can consider antifungal creams.  

If the foreskin is a persistent problem despite conservative treatment, like medications, then you can consider a urology evaluation for any procedure considerations.  

Followup with your personal physician is essential.
