I am a straight AA male. On May 24, 2008 I received unprotected oral sex from a female AA street prostitute in a predominately AA county with the 2nd highest HIV rate in my state. I have reason to believe that the guy she serviced before me, who finished in her mouth is HIV +. I too finished in her mouth, an had scrapes on penis from prior night's sex. Would infected semen of the guy, in her mouth, be a potential threat to me? This is the first time I have had sexual contact with someone other that my wife in 9 years.

About 2 weeks later I started to feel pins and needles in my face and groin. I went to the doctor who gave me a shot and antibiotics to take. They checked my urine and blood and tested everthing came back clear. I have HSV1 and thought maybe I had caught HSV2 so I started Valtrex, to reduce the possibility of a sever outbreak. Would Valtrex delay seroconversion if I was infected with HIV?

For the past seven months I have all sort of symptoms,   My nose, I can see sebum in the mornings, I can then squeeze and it will spiral out: I NEVER HAD THIS PROBLEM BEFORE.  Nausea everyday.

The symptoms that are really worrisome are swollen lymphnodes in neck and armpits, prostatitis returning, fungal infection beneath penis head, and my first ever abnormal blood test: my hematocrit is low. I have dizziness.

My wife does not know of this and we only have sex using condoms. BUT she has begun to have irregular periods as of about 4 months ago for the first time in her life.  WBC high, liver function not normal. A MRI recently showed her neck lymphnodes are swollen.  We both continue to have symptoms we never had before.

I tested negative for HIV, Syph and HSV2 on October 5...would this be conclusive? We still have new strange symptoms.

This is not HIV.  It could be some other sort of viral illness but to ascertain this you must be tested.  As I said, the way to detrmine this is to work with your doctor.  I have nothing more to say.  EWH

Dr.  I currently feel horrible:

I have confirmed swollen nodes in neck armpits and groin and swelling behind knees.
Dizziness, strange stool; sometime light colored, sometimes muddy
Red pimples that come and go, mainly on forehead
Skin seems like it is changing texture , darkening
Nausea, bloated, abdomen pain right side, just started this month.
When I get a paper cut or scratch as it heals, it darkens
Low body temp, constantly between 97.1 and 97.9


Could this be Hep? Doc is testing me for EBV and CMV?

Last time, you do not have HIV or STD.  Something else may be going on but it is not HIV or STD.  I will not say this again.  You and your wife need to work with your doctors to figure out what is going on but it is NOT STD or HIV.  EWH

My wife just had a lab report and her vitamin d level is low. FOR THE FIRST TIME EVER!

I have swollen nodes in my neck and armpits and the back of my knees burn, and low hamatocrit for the FIRST TIME EVER!

Could this be the first case ever from someone getting a **?

I have only had sex using condoms with my wife, since the unprotected **.

Dr. are you sure I am in the clear, this is just so strange and scary!

You do  not have an STD of any sort.  As far as what is going on, I think you need to continue to work with your doctors.  Sometimes these things take time and a certain amount of trial and error.  EWH

Dr. I tested negative at 5 months 1 week for HIV Syphilis and HSV2. In your experience have you come across a less common illness that one could get from insertive oral sex?

Should I test for Hep A B C or MRSA? My doctors don't know what is wrong.

Thi sis unlikely to be an STD-related problem resulting from the expsoure you describe.  I would suggest you work through it with your own doctor.

BTW, I neglected to answer your question about the valacyclovir.  The answer to that part of the question is that valacyclovir would not be expected to alter your test results in any way.  Take care.  EWH

Dr., is there anything besides HIV that I could have gotten from this incident, that would cause such wide spread problems? I had none of the problems before this.

We get many questions about the risk of HIV from receipt of oral sex on this forum.   There is some debate as to the risk of getting HIV from oral sex from an infected partner.  The figure we use is that the risk is 1 infection per 10,000 exposures.  There are others however who feel that there is no risk at all for HIV from oral from an HIV infected partner.  Either way, the answer to the questions is uniformly that the risk is very low.  These average figures hold even with modifying factors such as whether the person had sores in their mouth, gum disease or, as is your case, the person had just performed oral sex on another HIV infected person.

In your case, you have nothing whatsoever to worry about since you have a negative HIV test more than 12 weeks following your exposure of concern.    

So this now brings us to the question of swollen lymph nodes and other problems.  It is not normal to have swollen lymph nodes or a low hematocrit and you should work with your doctor to sort out what it causing them.  At the same time, one thing you do not need to be concerned about is HIV.  You did not get HIV from the exposure you describe.
