ultrasound results

I just have a mammogram and a follow ultrasound after close to 6 years or not to have any, I never has an ultrasound until now.  The doctors find microcalcifiacations long time ago, and I have 3 biopsys. The first biopsy was done in 1996 the other two in 1997, now I’m 49 years old.

All pathological reports from the 3 excisional biopsies show for the First one: microcystic changes, apocrine metaplasia, sclerosing adenosis and stromal fibrosis.
For the Second and Third,: proliferative fibrocystic changes of the breast exhibiting duct epithelial hyperplasia and multiple focus of intraductal papillomatosis Duct Ectasia. Sclerosing adenosis with microcalcifications.  Papillary apocrine metaplasia.. With the note: both right and left breast biopsy exhibits exuberant fibrocystic changes. Continued close clinical follow up is recommended.
I did two or three mammogram after that and I stop in 2002. Now the ultrasound report show:
“FINDINGS: The exam demonstrates some solid lesions that have a benign appearance in the right breast at the 9-10 o'clock position, one measuring 0.5 x 0.4 x 0.6cm and the other measuring 0.5 x 0.3 x 0.4cm. The left breast also shows a solid mass at the 3 o'clock position measuring 0.5 x 0.3 x 1.0cm. These have a benign appearance. There are some dilated ducts in the retroareolar region on the left.

IMPRESSION:
BIRAD category 3: Probably benign findings. Recommend 6 month ultrasound follow-up or clinical correlation.”
What is clinical correlation?, should I be concern after my previous results? What all that means? I know that with Birad 3 there is less than 2% chance of cancer, that is really good but should I get a second opinion or wait 6 months?, thanks

Dear digitec:  The findings from your previous 3 biopsies are benign.  Clinical correlation means that these results should be considered in the context of your clinical breast examination and your doctor’s opinion about the level of suspicion.  A BIRAD 3 means that there are some abnormal areas that appear benign to the radiologist, a 6 month follow up is recommended to be sure that these are stable and that they do not change in a short time interval.  If you are uncomfortable, you could obtain a second opinion with review of the ultrasound.

I suppose you could request that your films be read by another Radiologist but from the results you state and the BIRAD 3 rating the 6 month follow-up seems reasonable. Your films are always compared to the previous exams. With the previous results you've had I would have the recommended yearly mammograms though or at whatever interval the Radiologist recommends

I think I do not explain my problem correctly.
You said “. Your films are always compared to the previous exams. With the previous results you've had I would have the recommended yearly mammograms though or at whatever interval the Radiologist recommends”.
But I forgot to mention, that when I move from Spain to Miami some of my mammograms disappears, I have one biopsy in Spain (1996) two in Miami (1997).  After that I have few more mammograms, but all disappears. I don’t have a breast image until now March 2008, meaning that nobody can compare to previous ones.
When I move from Spain I keep the medical record and I made copies of some results like the pathology of the biopsies.
My medical record from Miami (1997-2002) disappears, I stop visiting my doctor or any other doctor in 2002. You’re maybe surprise it take me to long to see a doctor, but sorry I do not like to go see doctors I’m scare I panic when I see them.  I do not have image records of previous x-ray mammograms there all lost.
After my abnormal mammograms in March I do not have a previous mammogram, all my x-rays are lost.  I try to give them the written pathological results of the biopsy but, they do not want then.
You tell me that my films are compared to previous ones, sorry but mine has been lost for many years, even they do not have the written pathological results,  I know that they said is a Birad 3, but can I trust all that without been compared to previous exams?

I’m mad but is not with you, or the doctors, is with me and the system. With me for be afraid of doctors and take to long to see a doctor and not express myself and we the system for not have the right tools to secure my medical records.
