3 year old bowel movement

My 3 year old grandson will fight having a bowel movement for days.  He will go but it is very small.  He starts to shake  and will cry/scream very loudly for long periods of time.  He doesn't want to b left alone and wants to be held or have someone hold his hand.  It seems to happen every couple of weeks.  Is this an issue of him trying to control what is going on around him?

Probably not. He is constipated and the bowel movements are painful. His pediatrician can offer a regimen that will promote bowel movements and make them more comfortable.
