What is Grade I Anterolisthesis of C7 on T1 by 3 mm???

Hi Doctor,

I recently had a MRI on 2/4/10 and the findings from C4-C7 anterior cervical fusion were unchanged from my 9/26/07 MRI.  I had ACDF in January 1993 and December 2000.  I am fused from C4-C7 with titanium plating.

A NEW finding shows "Grade I Anterolisthesis of C7 on T1 by 3 mm".  

I am VERY concerned about what this is and if it happened due to my right shoulder surgery on 11/3/09 or physical rehab???  What is the cause and treatment for this finding???  Thank you for your time and consideration!  :-)>

Diane, "anterolisthesis" is a term used to describe abnormal forward/frontward movement of one vertebra over another. Your MRI finding shows the seventh cervical bone moving forward atop the first thoracic bone by a measurement of 3mm.

Typically this happens as a result of fracture. There's really no "treatment" other than surgical correction. Your physician may not elect to advise surgery until the movement reaches a certain distance.

Hi Dr. Tanase, it's Diane again!  
     I need your expertise again.  I went to my psychologist today because of anxiety and frustration about my C7-T1 slippage and relentless pain.
    She helped me to get 3 questions composed to ask my Orthopedic Surgeon who performed my Right Shoulder SAD, Mumford and Debridement of SLAP lesion on 11/3/09. At my first post-op visit, he stated that I was doing well but that I had increased neck discomfort and trapezial discomfort.  
    I have been going to physical therapy but had to stop for two weeks because of the chest pain and increased right-sided pain in neck, shoulder, bicep, tricep, deltoid down to top of my palm and into ring finger and baby finger.  This prompted the MRI which I had done on 2/4/10.  The anterolisthesis at C7-T1 IS A NEW FINDING compared to past MRIs (1993-2009).  I am really scared!
    First question is, did the physical therapist know of my C4-7 fusions, weakness and limited ROM?  Second question, did the physical therapist know the appropriate protocol to follow on my P.T. prescription?  Third question, why was I pushed past my comfort level pushing and pulling 30 lbs. of weight? Did he review my x-rays, MRIs and reports?
    My questions for you are . . . do you think that treatment with shoulder P.T. and Ibuprofen are worsening my cervical spine???  Will shoulder P.T. worsen and/or cause further movement of C7 on T1???  What is the specific distance before I have to decide on "surgical correction"???  The past week, I am wearing my soft cervical collar and using moist heat and Ibuprofen for pain relief.  I can barely hold my head up.  I am also very disappointed with the results of my surgeries.  I can't handle living so dysfunctional and useless.  Are my expectations and goals too high???
    I would be sooo grateful for any advice or guidance from you!  I trust you!
