Very Painful Urination

This has happened about 3 times lately, all three times were a month apart and the most recent was yesterday. I will have a painful unination and white stuff is in the toilet its a mass of some sort i have been put on antibiotics and it goes away for awhile then about 2 weeks after taking the anitbiotics it comes back. I have been checked for gonnoreah and chlamydia and both came back negative. what could this be? any information is helpful. and this mass is soft it's not hard like a kidney stone
