high output heart failure

Hi,
I am 45 yr old female who has highoutput heart failure on and off secondary to anemia.due to a partial gastrectomy for gasrtro paresis when I was 19 yrs old. due to gastric ulcers.I also had congentital bicuspid valve which was stenosed.Then I got endocarditis which further destroyed my aortic valve so bad it had to be replaced with a mechanical valve.I have been on coumadin since mt surgery in March but there has only been three weeks myINR has been within therapeutic range 2.5-3.5.I am currently taking 10mgs of coumadin daily at dinnertime,but my INR is still only 1.9.
I am concerned about a clot and lately I have been lightheaded and dizzy and I am always tachycardic.My question is what symptoms would I have if a clot was to start forming because I could not go through the operation again.I am aware of food restrictions,take my coumadin at the same time everyday and have not missed any doses.
please advise.

The symptoms would depend on the location of the clot.  If the clot is in the pulmonary vessel and/or lungs there would be high pulmonary vessel pressure and shortness of breath, right side heart failure, chest pain, etc.  If a coronary vessel becomes blocked, there can be chest pain, heart failure, heart arrest. A clot to the vessel going to the brain can cause stroke, coma, death as well a clot to the brain.
