Dry socket--neighboring tooth trauma?

I'm wondering if a dry socket can cause pain in the neighboring tooth . . . I had a lower molar extracted a week ago by an absolute a** of a dentist (didn't believe me when I said I wasn't fully numb and begged him to stop, called me hysterical, and the sum of my aftercare was a prescription for two Tylenol 3s and instructions to pray to Jesus to take away my pain).  Four days later, excruciating pain sent me first to the ER and then to a much nicer dentist who diagnosed me with a dry socket, packed it with the clove gauze and prescribed Vicodin.  I'm due to have the gauze removed tomorrow (dentist said nothing about re-packing).  The pain was alleviated a *lot* the first day after the packing treatment, but has returned somewhat today, and I've noticed that the premolar neighboring the extracted molar has become sore--it hurts when I bite down or tap firmly on it.  Could this be from the dry socket issue, or is it a more serious trauma?  I've taken my antibiotics religiously and the second dentist said the X-rays showed no infection, and previous X-rays and exams showed no serious decay or other issues with the premolar that's now become painful.

Sorry you had such a bad experience, just remember not to go back to the first dentist.  Some of us are actually nice people ;)

If you are still in pain, ask the dentist to repack another gauze.  Sometimes it takes up to a week or two for the pain to finally resolve from a dry socket.

Now, your first molar (the extracted one) carries a majority of the biting force.  By removing it, you have transferred that force to neighboring teeth, which were not designed for that much (like the premolars).  There are several options.  First, try to eat on the other side for a few weeks to allow the extraction side to heal and the premolar to adjust.
Ideally, you will want to replace that first molar with an implant, to put the biting forces back into correct function.  A partial denture would be a secondary option.
