do cavities form from the inside out?

Cavities are decay that is forming in the enamel of the teeth. If the enamel was softening already, a cavity is forming. When examined, many times the softened enamel becomes dislodged showing the cavity.
I don't think your dentist poked holes in your daughter's teeth, but rather revealed the areas that a cavity is forming. Depending on number, place, depth and area to cover, your dentist may be charging accordingly.
Please ask your dentist to clarify his/her treatment plan so that your are satisfied the work should be done.
