dentist won't give refund for crown

i had a root canal done a little over two years ago. the root canal was successful, but the crown broke three months after. the dentist (let's just call him dr. R), offered to put a new crown on, but that one was ill fitting. i could feel the tooth move around when i chewed, and saliva would get underneath the crown and cause bad breath. in addition, the badly fitting crown made my gums bleed and food would get in-between the crown and the teeth surrounding it. dr. R re-cemented the crown back on but it fell off several more times in the next year. finally, he offers to re-make the crown again but once again this crown was ill fitting and fell off again. it needed to be re-cemented again several more times. finally, i become fed up and go to his senior partner (dr. M) at the same dental facility, who is older and more experienced. the dental practice is named after him so i thought that he would be able to fix what his more inexperience partner had messed up.

dr. M says that i no longer have any tooth material for the crown to hold on to, and recommended that i get a dental implant. he told me over the phone that the reason i needed an implant was because dr. R had taken off too much of the original tooth material when he made my crown and that there wasn't enough material for the crown to attach to. i paid around 900 for the original root canal, over 1000 for the crown, and now i might have to pay another 1500-2000 for the implant.

dr. M has offered to cover the cost of the new crown after i get the implant, however I would just like a refund on the over $1000 i paid originally because i feel like they've already messed up my crown three times due to a mistake on their part (taking off too much original tooth material so that the crown now has nothing to adhere to), i would just rather go to a dentist who knows what he's doing. i called dr. M and explained my situation, and basically asked if i could get a refund for the crown. he became argumentative and said that they have already paid more than $1000 out of pocket by cementing the tooth on for me over and over, and re-making the crown three times. he said that he offered to do another crown for me after i got the implant done at a specialist 'out of the kindness of his heart' because he felt sorry that i was a broke student. i feel that he is making it sound like he's doing me a big favor when he is only offering to do what i paid him to do originally: give me a crown that stays on. the call ended with dr. m saying that he is going to go over my dental records and think about it.

MY QUESTIONS:

1. can dr. m change my dental records so that if i file a dispute with the state dental board, the case will be in his favor? i noticed at my last apt. that the assistants basically type in a little description into a box, and i was wondering if he could go in the program and just edit what they had written? i want to ask for a copy of my dental records so that i could have proof of the numerous times i've gone in to see him about a problem with the same tooth.

2. i called the illinois state dental board, and they said that they only handle disputes regarding work that has been done in the past two years. the work on my tooth (root canal, original crown) was done over three years ago but since then they've been doing maintance work such as cementing the crown on and re-making the crown every few months or so for the last three years. do i still have a case?

3. dr. m basically said to me over the phone that it was because dr. R had taken off too much original material that caused the crown to not be able to stay on properly. however, i don't have any proof he said this and i doubt he would admit to saying it. in the same conversation (after he told me i needed an implant), he also said that sometimes crowns just don't stick, and sometimes a dental prodecure doesn't always go as planned. he said it could be possible that my tooth just wasn't going to 'take' the crown. however, if dr.R decided to give me a root canal and crown, then i would have to assume that he used his professional judgement and believed that the tooth was going to 'take' the crown, or else he would have gone a different route. in fact, he believed it was the proper procedure that he did TWO MORE crowns after it. and then when i saw dr. m, he must have believed it was the proper prodecure too because he kept on cementing the crown back on over and over again. if they both were under the assumption that the tooth wouldn't 'take the crown' like dr. m is claiming, then why did they spend the greater part of two years trying to cement the crown back on and making me new crowns? and logically, if i paid for a crown to be put on and they haven't been able to put one on, doesn't that mean that i get a refund on what i paid for? i paid for the crown, but i don't have a proper one on. therefore, refund?

i know this is long, and i appreciate any help you might have.
