O.c.d. together with bipolar disorder

I am a pharmacist 28 years old male suffering from typical o.c.d. which disturb my life starting at 15 years old I think.
while psychiatric therapy is ashamed and expensive in my country Egypt I fight the thoughts and analyze It mentally as I can ignoring that I have a real problem for at least 10 years before first time visit a doctor who easily diagnose the case as o.c.d. and prescribed me on anafrnil75 mg "Clomipramine"
but in this 10 years I faced depression during my endless fights with repeated bad thoughts at intervals also some times normal others hyper reactivity occur.
but because it was not regular episodes yet the doctor did not notice that h suffer from cyclic mood case also
but he gave me tegretol "carbamazepine" also when I started to feel afraid of being well and it was working for a period of time
but about the o.c.d. >90 percent disappears from the first few days after Clomipramine using while its side effects was very bad
but I am on it"Clomipramine" up to now
but after about one 8 months of its first dose i start to face regular depressive episode every 10-13 days and last for about five days with too excessive sleeping then suddenly recovered to an excitement period for about 3-5 days with very little sleep and hyper physical & mental reactivity followed with a normal mood period last for 5-7 days before the cycle repeated again
first doctor give me tgretol "carbamazepine" which slightly work .then i tried lamictal "lamotrigine " for a while and stopped it due to headach appeared
and the last year i he try ecitalopram with me before finally put me on faverin" Fluvoxamine "together with clomipramine which i never stopped although i tried many times with every new combination medicine as the o.c.d.  relapses every time and the cyclic attacks never stops with any medicine while the doctor advise by taking dogmatil 50"sulipride" 2-3 times daily during the depressive episode slightly help
in fact my doctor diagnose the case as o.c.d. with cyclic depression while I realized that it is o.c.d in combination with bipolar type 2 disorder
the question is did I finally diagnosed the real case correctly and if so what is the suggested medications should I take for this combinative case .and how can i exchange clomipramine with more save and less side effects drug

I have schizoaffective disorder with the bipolar aspect and I had the same problem with Anafranil. An anti-depressent can potentially worsen the symptoms of mania in a person with bipolar. But if you believe that to be your diagnosis speak to your psychiatrist about it and also keep a mood tracker here so they can better understand what is going on.
