Black Spots around hair folicles on scrotum

My boyfriend has black spots around the hair follicles on the scrotum on one testicle. They are a bit raised, itchy, slightly scabby and extremely painful. The affected area seems to be getting slightly larger every day. He rides motorcycles and has had this "rash" since he went on a particularly long ride 5 days ago. We have not used anything to treat it topically, except last night before bed, when we used a herbal baby bottom salve. Any idea what is going on down there?
