Calous ring around foreskin

Since the beginning of this month I've had a calous starting to form around the top of my foreskin right beneath the head.
It has caused me some pain and itching and have seen my doctor over 4 times since this started.  
I also cannot have sex due to the pain it causes from rubbing against anything.  

My doctor prescribed th following and currently I'm only using the Clotrimazole-Betamathasone cream.

8-04-2010 – Prescribed Nystatin
8-08-2010 – Miconazole
8-10-2010 – Prescribed Fluconazole, 200mg & 100mg
8-15-2010 – Lotriman Ultra
8-18-2010 – Prescribed Clotrimazole-Betamethasone

My doctor also ran a full scan of tests to rule out any STD (all were negative).  All my other labs are normal also and although I'm HIV+ none of my results have been flagged for anything.  An STD Specialist also looked at my issue and had no idea what it could be but ruledout herpes, etc.

Its becoming very frustrating to say the least.  I'm 53 years old and before this I have NEVER had any problems with my foreskin.  It also fully retracts to the extent that no one knows I'm uncircumsized when erect.

I do have an appointment with a dermatologist on 8/31/2010 but I still can't get an answer my only option is to see a Urologist.

I've done extensive research on line and cannot seem to find anything about this issue what-so-ever.  Any ideas would be welcome.

Hello,
Form the symptoms the possibilities of lichen sclerosus and balanitis xerotica. Lichen sclerosis appears as white, dry plaques on the glans (glands), foreskin or shaft of the penis. Balanitis xerotica is a severe form of lichen sclerosis. Psoraisis (psoriasis) can also cause dry ,itchy skin rashes.

In my opinion a clinical examination by a dermatologist will help in confirming a diagnosis as treatment depends on the same. Please get it done.

It is very difficult to precisely confirm a diagnosis without examination and investigations and the answer is based on the medical information provided. I did see a Dermatologist on 8/31/2010 and it appears to may have started (in her opinion) as contact dermatitis with subsequent secondary impexiginizuxier.  She prescribed Cephalexin 500 mg capsules and Nystatin/Triamcinolone Topical ointment.

Since I started treatment 8/31, the calous is softening and the pain has diminished yet I have had a high fevor (1st night 101.4, last night 99.9) and sweating throughout the night.

I see the Dermatologist on 9/8/2010 for follow-up and hope that before then this clears up more.
