Rash and gentals on rear that won't go away

I've had a rash on my rear for over two months. It has been (7 or so around the crack) raised pinkish red spots (pencil eraser size and a bit bigger) on my but that itch a lot. Several seem to be going away but are still smaller flat red sports. I also have had spots that itch on each side of my scrotum (now just one) that become raised and red when I itch them. These have all just been pinkish red spots. A nurse, though, once said one on my rear had a small whitehead.  

Two weeks ago I developed a pink spot on the tip of my penis that is very sensitive. It doesn't really hurt but is sensitive. I now have another flat pink spot on the other side of the tip.  

I have also had itching on my chest and forearms with some redness on each.  In the last few days my neck on the front and back has been itching. Now I seem to have small white heads that itch on my neck.

I'm on my second antibiotic and have taken acyclovir. My primary care doctor ran herpes and syphilis tests and I came back poitive for HSV1, though he seemed to think the symptoms weren't consistant. In the last couple days I've started using Lamisil as well just to cover my bases. My doctor recommended a topical steroid for the bumps of my penis. But he seems to be treating those as they are different than the rest. I somehow doubt they're unrelated.

I also have guttate psoriasis and have little on my shins that came on several months ago after a strep infection. But I did light treatments and that kept it from being widespread. I have no idea whether that's related. I've never had psoriasis itch like this.

My primary care doctor has looked at this a few times as has a nurse, but I can't get into see a Dermotologist until Oct. I belong to HMO that seems excellent at gatekeeping.  This is starting to drive me crazy and I want to make sure I'm not trying anything that might be making it worse.

Your list of symptoms is bewildering.  The spots sound more like irritation or psoriasis than any STD.  My advice is to use only non-medicated moisture creams, to leave the lesions alone, and to show the dermatologist what (if anything) is left on October 1st.  If the dermatologist thinks it's psoriasis, a topical steroid cream may be the recommendation, but I would wait on using that until you are examined.

I've also had itching on my thighs. Though the neck irritation seems better today. I've also had tiny bumps on my upper arm with tiny white heads.

I stopped taking the antibiotics yesterday. They seemed not to be helping and if this is a fungal infection it seems like they might actually be hurting.

I've been very leary about using steroids as well as I'm afraid that might make it worse. Currently I'm just using lamisil, but I can't really tell if it's helping.
