Continuing issues with toes

Ive posted here before about my toes. For years now I have been dealing with swollen toes. They itch, they hurt, and they always look bad, even when they aren't flared up. I get these flare ups often and nothing I try makes them go away. And Ive tried everything. Dermatologist did blood work and biopsy and I had a positive ANA. And she kept mentioning lupus. I haven't been diagnosed with anything yet. It's been a bit since Ive been there and need to go back soon. It just gets costly.
I've tired keeping them warm, keeping them cool, I've truly exhausted all things. Looking back now, I've been having issues with my toes since I was 16 years old. I just never thought anything of it. The sides of my toes would swell up near my nail back then. It's gotten much worse since then.  I hate posting photos because its embarrassing but it may help. What does this look like to you? Please help.
