Sore Itchy Discoloerd hands and back

My mother (age 66) has had this rash that started on her hands. Her hands are two toned the bottom half being light and the part by her fingers are dark. She is also complaining of aching in her hands. Her hands are so sore that that if you brush them it hurts her. The rash has also spread to her back and face. There are no bumps on her hands but on her face and back the bumps are present. She is seeing a dermatolgist who had prescribed several cremes and apills to ease the itch but nothing is working.
Do you have any idea what this could be and how we can help her because it has caused her blood pressure to rise?
