Pain in the back or lumbar region when you don’t urinate for some time is indicative of kidney causes like kidney stones. Given your history, you need to get an ultrasound kidneys and your creatinine levels done.
Please elaborate the symptoms and the description of the dark spots in detail so that I can help you in a better way by writing about the differentials which can cause the symptoms.
Mention about the color, size, shape, associated symptoms like pain, itching, discharge etc if you have any.
