foreskin cracking

hello doctor
im a 25 yr old man and for the past 4 month's the skin on my penis has been cracked at the tip.
im not circumiszed and i have diabetes. i've tried all types of creams and anti biotic but nothing is working.
no discharge when i pee just crack skin at the tip of my foreskin not on the penis head. please help me because its discouraging and embrassing. it seems like when its heal and i try 2 pull the foreskin all the way back it cracks up again. is there any other kind of medicine i can use?
